New Customer Forms

 

Since we do not sell to the general public, any new customers we take on must be approved before they can buy products from Hillcrest Garden Inc. All forms below must be completed, submitted and approved before a customer can be approved to purchase. Once a customer is approved - we can set up logins and passwords to buy flowers and supplies through our online portals. All purchases are assumed to be for resale (non-taxable with tax ID number) or taxable (if the items are only purchased for display). We qualify each potential customer to insure that we are not selling directly to consumers. This helps us to protect the traditional wholesale and retail channels of distribution.

To become a customer of Hillcrest Garden Inc. bring the following documents with you if you visit us (or you can FAX the information to 201-599-9903, or email them to criley@hillgar.com).  All required forms are at the bottom of this page. 

  • A photo copy of the business's Tax ID Certificate which shows the state retail tax number.
  • Customer Information Application.  Completed in full and including business address, business phone, owner's name, and owners contact information.  This document requires a signature on the final page that must be signed by the owner in a legible fashion.  Failure to complete the application in full will result in account restrictions.
  • Certificate of Resale.  Completed.
  • Photo ID
  • To open credit with us, please complete the bank details on the Customer Information Application.
  • Credit is not required to become a Hillcrest Garden customer. 

 

Applicable taxes for each state will be added to the purchase price of  taxable items as a default.  Taxes will not be removed until exemption requirements are met.  If eligible for reimbursement, taxes will be reimbursed only after invoices are paid in full and all required documentation (such as a tax ID form) has been received.  Required forms may be faxed to 201-599-9903 or emailed to criley@hillgar.com

New Jersey:
Resellers: A sales tax exemption is allowed for companies purchasing for the purpose of resale.  New Jersey Buyers must submit the NJ form ST3.  Non-residents must submit the NJ Form ST3-NR.

New York:
Resellers: A sales tax exemption is allowed for companies purchasing for the purpose of resale.  Exemptions will be allowed for qualified resellers with the completion of NY ST120.

Resellers: A sales tax exemption is allowed for companies purchasing for the purpose of resale.  Exemptions will be allowed for qualified resellers with the completion of Connecticut Sales & Use Tax Resale Certificate.

Resellers: A sales tax exemption is allowed for companies purchasing for the purpose of resale.  Exemptions will be allowed for qualified resellers with the completion of Pennsylvania Form REV-1220.
